Full-time workers (employed more than 20 hours per week and intended to work for more than 5 months) are enrolled in the Concordia Disability & Survivor Plan (CDSP). The CDSP offers both disability benefits for workers and lump-sum death benefits to workers and their enrolled dependents. The amount of the benefits are determined by the individual worker’s circumstances.
Worker without Dependents - 1.20% of total annual compensation
Worker with Dependents - 2.25% of total annual compensation
Reporting a DeathTo report the death of a worker or dependent, call Concordia Plan Services at 888-927-7526. Once we have received your call we will send a packet of information including forms to complete. Along with the completed forms, a certified death certificate is required.
In the event that a member dies while overseas, the death needs to be reported to the U.S. Embassy in the country in which the death occurred. Additional documentation needed may include: police report, physician’s statement, certified death certificate, and cancelled passport.
